The purpose of this form is to send your withdrawal request.

Sending the form: You can complete the form online, and we will notify you by email upon receipt.
You can also send us this withdrawal form by email, fax, or registered mail, accompanied by your signature and the date of dispatch (the date of the letter, email or fax being taken as proof).
Product return: You have a maximum of 14 days after your withdrawal request to send the products concerned.
Returned products must be in original condition, accompanied by their original packaging.
Refund: Refund will be made within 14 days of receipt of the products in original condition.
